At its core, MTM allows you to work directly with a pharmacist to better understand the medication you’re taking, review your medication record and develop an action plan for your medication therapy moving forward.
It’s completely free and our patients have found it to be incredibly insightful.
Here’s how it works
Once a year you come in and sit down with one of our pharmacists here at Halifax and we talk through all the medications you’re currently taking.
From there, we educate you on how you should be taking them and look for other medication options that could be more effective from a cost standpoint or for your overall health.
It’s as simple as that.
